Radiology. 2020;192018. doi:10.1148\/radiol.2020192018<\/p>\n<p>Link: <a href=\"https:\/\/doi.org\/10.1148\/radiol.2020192018\"><u>https:\/\/pubs.rsna.org\/doi\/10.1148\/radiol.2020192018<\/u><\/a><\/p>\n<p>Nesse estudo prospectivo realizado por Lindenburg et al, os autores buscaram a valida\u00e7\u00e3o histol\u00f3gica do uso do agente tra\u00e7ador do PET PSMA 2-(3-{1-carboxi-5-[(6-[18F]fluoro-piridina 3-carbonil)-amino]-pentil}-ureido)-\u00e1cido pentanodioico (18FDCFPyL) para detectar recidiva do c\u00e2ncer de pr\u00f3stata em pacientes com n\u00edveis crescentes de PSA e comparar esses resultados com a RNM multiparam\u00e9trica (mpRNM). O 18FDCFPyL \u00e9 um agente marcado com fl\u00faor do PET PSMA que demonstrou alta afinidade por doen\u00e7as malignas da pr\u00f3stata, mostrando-se ao mesmo tempo promissor para a identifica\u00e7\u00e3o da doen\u00e7a metast\u00e1tica ou c\u00e2ncer de pr\u00f3stata residual. Foram detectadas 323 les\u00f5es por meio do 18FDCFPyL e da mpRNM em 77 homens com recidiva bioqu\u00edmica ap\u00f3s prostatectomia e\/ou radioterapia. O 18FDCFPyL teve um desempenho melhor do que a mpRNM na detec\u00e7\u00e3o de linfonodos p\u00e9lvicos (128 vs 23 linfonodos) com uma concord\u00e2ncia de detec\u00e7\u00e3o de 25% (82 de 323). A escassez da literatura atual e os desenhos heterog\u00eaneos de v\u00e1rios estudos sobre resultados a longo prazo da crioterapia para o tratamento do CCR cT1 levaram os autores a avaliar a sobrevida nos pacientes que realizaram crioterapia guiada por imagem para les\u00f5es cT1 em sua pr\u00f3pria institui\u00e7\u00e3o. Eles avaliaram a sobrevida global (91%, 86%, 78% e 76%), a sobrevida livre de doen\u00e7a (94%, 91%, 88% e 88%), a sobrevida livre de progress\u00e3o local (97%, 95%, 95%, 95%) e a sobrevida espec\u00edfica da doen\u00e7a (99,6%, 99%, 99% e 99%) aos 3, 5, 10 e 15 anos, respectivamente. Dois radiologistas especialistas em abdome avaliaram as les\u00f5es de forma independente e em consenso, obtendo uma concord\u00e2ncia interobservadores com coeficiente de Kappa 0,57 para a vers\u00e3o original e 0,55 para a vers\u00e3o 2019. Os resultados do estudo mostraram que, no geral, a sensibilidade e a especificidade de identifica\u00e7\u00e3o do carcinoma de c\u00e9lulas renais (CCR) foram 100% e 86% na vers\u00e3o 2019 e 95% e 81% na vers\u00e3o original dos crit\u00e9rios de Bosniak. Os pacientes foram submetidos a mpRNM 3T com e sem ERC durante a mesma consulta. As imagens foram avaliadas por dois radiologistas de forma independente, os quais n\u00e3o determinaram diferen\u00e7as significativas na qualidade da imagem entre aquelas obtidas com e sem o uso da ERC, com base em um sistema de 5 pontos (com ERC 3.8 vs sem ERC 3.5). A \u00e1rea abaixo da curva para a detec\u00e7\u00e3o de c\u00e2ncer clinicamente significativo (escore de Gleason igual ou superior a 7) entre os observadores individuais foi de 0,96 e 0,96 com ERC e de 0,88 e 0,91 sem ERC. 4 de 13 tumores com escore de Gleason 3+4 n\u00e3o foram detectados sem ERC, enquanto 1 de 5 tumores com escore de Gleason menor que 3+4 foi indetect\u00e1vel com e sem o uso de ERC. A TFG de cada rim foi calculada utilizando um fator de proporcionalidade derivado do produto do volume renal e do n\u00famero de incrementos na TC determinados pela TC multif\u00e1sica (sem contraste, arterial, venosa e fase nefrogr\u00e1fica). A TFG de um \u00fanico rim gerada em cada fase foi comparada com a TFG pelo m\u00e9todo Gates, calculada a partir de imagens din\u00e2micas renais com \u00e1cido 99mTc-dietilenotriamina penta-ac\u00e9tico. Em seguida, os autores calcularam os coeficientes de correla\u00e7\u00e3o das fases sem contraste (0,729), arterial (0,781), venosa (0,788) e nefrogr\u00e1fica (0,842), mostrando que a fase nefrogr\u00e1fica se correlaciona melhor com o menor desvio m\u00e1ximo de \u00b136,9% da TFG calculada pelo m\u00e9todo Gates. Foram inclu\u00eddos no estudo artigos com mais de 10 pacientes submetidos \u00e0 avalia\u00e7\u00e3o diagn\u00f3stica por RNM com achados histopatol\u00f3gicos comprovados e consistentes com lpAML. 2196 pacientes foram avaliados em 23 estudos. A sensibilidade, especificidade e a \u00e1rea abaixo da curva combinadas foram de 83%, 90% e 0,93, respectivamente. Dois observadores, os quais n\u00e3o tiveram acesso \u00e0s informa\u00e7\u00f5es cl\u00ednicas, analisaram as imagens de 36 pacientes com c\u00e2ncer de pr\u00f3stata comprovado por bi\u00f3psia, comparando sequ\u00eancias DWI obtidas por imagens eco-planares de tomada \u00fanica (ss-EPI), imagens eco-planares de transmiss\u00e3o paralela (ptx-EPI) e imagens eco-planares de m\u00faltiplas tomadas com leitura segmentada (rs-EPI), todas adquiridas durante um \u00fanico exame. As sequ\u00eancias DWI foram obtidas com os seguintes valores de b: 0, 500, 1000 e 1500. Ss-EPI apresentou maior rela\u00e7\u00e3o sinal-ru\u00eddo para imagens b1000 e b1500, enquanto que a rela\u00e7\u00e3o contraste-ru\u00eddo foi maior em rs-EPI em imagens com alto valor b. A magnitude m\u00e1xima da redu\u00e7\u00e3o do ADC para o c\u00e2ncer de pr\u00f3stata comprovado por bi\u00f3psia foi em rs-EPI. Embora o ss-EPI forne\u00e7a a melhor rela\u00e7\u00e3o sinal-ru\u00eddo, o rs-EPI e o ptx-EPI tiveram melhor desempenho em rela\u00e7\u00e3o \u00e0 intensidade do contraste.
